Privacy and secure document handling.

This notice explains the information used by the Credible Credentialing website and client portal. Service agreements and any applicable Business Associate Agreement provide additional terms.

Information we receive

The public website may receive information you choose to send by email. The invite-only client portal receives your authenticated email address, uploaded file names, document categories, file sizes, upload dates, and the documents you submit.

How information is used

Information is used to respond to inquiries, provide contracted credentialing or practice-support services, authorize portal access, organize submitted records, and maintain the security and operation of the portal.

Portal access and document security

Portal access is limited to approved client email addresses. A client can access only documents associated with that client’s authenticated account. The Credible Credentialing administrator can review client submissions for service delivery. Documents are not intended for public access.

Sensitive information

The secure client portal is intended to receive requested provider and practice credentialing records. Those records may include dates of birth, Social Security numbers, Tax ID/EIN information, professional identifiers such as license, DEA and NPI numbers, malpractice information, W-9 or tax documents, and EFT or banking documentation. Clients should submit only the minimum information needed for their credentialing engagement.

Do not submit patient records or patient protected health information, passwords or account login credentials, or unrelated sensitive personal records through the portal. Sensitive credentialing documents should not be sent through the public assessment form or ordinary email.

Retention and deletion

Document retention and disposition depend on the applicable engagement, contractual obligations, and legal requirements. Clients may contact Credible Credentialing to ask about records associated with their engagement or to request appropriate deletion.

Third-party services

The website and portal rely on technology providers for hosting, authentication, and storage. Those providers process information as necessary to operate the service and are subject to their own terms and privacy obligations.
